Infographic - Children’s Comprehensive Care - Whole Child Visit Roadmap
Client: Children’s Comprehensive Care (CCC) is a primary care clinic dedicated to the care of children with complex medical issues. The clinic’s mission is to provide holistic healthcare to medically fragile children as well as their families.
Challenge: In response to insurance requirements as well as the needs of other healthcare services, the CCC is changing the approach to Well Child Visits. Children seen at the CCC are not “well,” therefore we changed the name of the visits to Whole Child Visits (WCV). Whole Child Visit considers the needs of the child on multiple levels—medical, behavioral, psychosocial, and educational, as well as, their hopes and dreams for the future. In order to communicate this paradigm shift in care, the CCC needs to explain the new model of WCV to the patients’ families.
Solution: An infographic called the Whole Child Visit Roadmap outlines the steps involved in a child’s twice-annual check-ups. There is a lot of information to digest. I wrote it concisely. From scheduling the WCV appointment to preparing for multiple specialist participation; from writing goals for the future to having a care plan with the next steps, the WCV Roadmap covers it all sequentially and succinctly.
